Parents, please fill this form out once for each of your students. If you have multiple students at the high school, please fill the survey out multiple times. This is for HIGH SCHOOL students only, grades 9-12.
Please note that this form is for data collection purposes only, and does not guarantee your enrollment choice. Your answers will be used to help us make decisions on what is in the best interest of all of our students, with more information to be released later on.
